The Maine Immunization Program and the Department of Education have been working very hard to streamline the process for conducting school located vaccine clinics in the 2011/2012 school year.

 Below is the link to the Maine CDC website with the updated forms for school located influenza clinics for the 2011/2012 school year.  It is the same link as last year, but the documents have been updated. 

 These are the forms to use for this year’s clinics.  If you have old forms from previous years, please replace them with the new 2011/2012 forms.
